DOT Guideline Updates

The Federal Motor Carrier Safety Administration (FMCSA) updated their policy for applicants with certain medical conditions or medications, effective February 2024.

This new policy requires applicants with the following conditions to have specific medical forms completed for their DOT physical:

  • Treated with insulin for diabetes
  • Non-Insulin treatment for diabetes
  • Diagnosed with “monocular vision,” as defined by FMCSA, or did not meet FMCSA’s vision standard at a physical qualification examination.
  • Taking medication(s) that may impair his/her ability to safely operate a Commercial Motor Vehicle (CMV)

These forms must be completed by your prescribing/referring medical provider before arriving at Fastlane, in order for the FastLane medical provider to complete your DOT physical.

Failure to have the forms completed prior to arrival at FastLane will result in an incomplete physical.

These forms are listed on the Lane website under patient forms on the Occupational Medicine page.
