Members of Lane’s Medical Staff will provide physicals free of charge on this date only.  Student-athletes who do not complete their physicals at this time will have to see a physician at their own expense.

            Student-athletes should bring a copy of the LHSAA Medical History Evaluation form with all medical history information completed including a parent/guardian signature.  Forms must be completed and signed by a parent or guardian or students will not be examined by the physicians.  Copies are available at the school.
